W-9 Request for Taxpayer Identification Number and Certification

Registration as a Lansing Community College vendor is not complete until
you submit a W-9 Form.   Please download and complete the W-9 form and return by email to lcc-purchasing@lcc.edu, fax (517) 483-5289 or mail to the address below.
